1️⃣ Always the alternative diagnoses should be excluded
قد يكون DLOC لسبب ٱخر مثل
👉 Intracranial bleed
👉 #Hypoglycemia (common in hepatic failure due to absence of glycogen stores)

💥 واتذكر مرة كيس اجتنا احالة من مستشفى برفري على اساس Hep. Encephalopathy
من قسنا RBS طلع 30 ..
وتوفت المريضة بعد ساعات للأسف

2️⃣ Always search for the precipitating factor
هنا تسأل نفسك هذا المريض جان
Compensated liver cirrhosis
هسه ليش صار Decompensated ؟

🔥الأسباب المهمة مثل
👉Infection (look for)
👉UGIB
👉Hypokalemia (ex. due to overdose of diuretic)

✅ لأن العلاج الرئيسي هنا هو
Treatment of the underlying cause

3️⃣ ملاحظتين مهمات عن الفلود 👇👇

🗂️ Normal saline #safe in cirrhosis
عكس ما تعلمناه

❌ Ringer lactate should be avoided
(بالحالة الطبيعية هذا lactate يتحول من خلال الكبد الى HCO3 وبالتالي يقلل Acidosis
أما بحالة cirrhosis راح يتراكم هذا lactate ويزيد acidosi
