Идиома значит "на последнем издыхании", и может легко применяться и в контексте вещей (что звучит более позитивно).

- runny nose - насморк

Можно перевести как "[доводить] до совершенства".
Например: «The English have raised the simple act of making a cup of tea to a fine art».
